Shift Prem. On Call Form This form is used to submit any Call Back or On Call shifts for payment.
Name Change Request Form Submit this form along with two pieces of identification to change your name with Carleton University.
Banking Change Request Form Submit this form along with your new direct deposit advice, or a void cheque.

Employee Documentation Forms

If this is your first employment assignment at Carleton University, please fill out the below forms, and hand in to HR within a week of your start date.  Please review the TD1 FAQ if you need any assistance filling out the forms.  We recommend that all documentation for new employees dropped off in person for verification of your SIN and Date of Birth.  Any delays in receiving the forms and verification will result in a delay in pay.
