Instructions: Complete the fillable course approval form and send to the e-mail address identified on the form (i.e., undergraduate forms to SLALSUndergraduate@cunet.carleton.ca; graduate forms to SLALSGraduate@cunet.carleton.ca).
Note: Only forms received from a Carleton e-mail account will be processed. Please include your name, student number, and the course in your message. For ALDS and LING requests, also include the name of the supervisor.

The purpose of this graduate supervision form is to create and maintain an accurate record of thesis/research essay supervision for graduate students in SLaLS. This information is helpful for grade submission, thesis defences, PhD progress reports, among other uses.
