General
- What is a COVID-19 screening self-assessment?

One of the ways that we can all play a part to ensure the health and safety of the Carleton community is by completing a COVID-19 screening self-assessment, an important tool in the effort to reduce the spread of COVID-19.
As part of the COVID-19 screening – via a self-assessment – a series of questions are answered regarding potential exposure to COVID-19. These questions help identify whether an individual’s symptoms are consistent with COVID-19.
- Is a COVID-19 screening self-assessment required before coming to campus?

COVID-19 screening self-assessments are no longer required. If you are feeling unwell, protect your colleagues, classmates and the Carleton community by staying home. You are required to submit a COVID-19 Symptom Reporting Form through cuScreen. Environmental Health and Safety or Health and Wellness Services will be in touch following the report to assess your circumstances and advise you on appropriate isolation timelines.
- How do I complete a COVID-19 screening self-assessment?

A secure and easy way to complete the required COVID-19 screening self-assessment online is by using cuScreen.
- How do I use cuScreen if I do not have a MC1 (Carleton) account?

If you do not have a Carleton University MC1 account, you must create a visitor account to complete Carleton University’s vacation attestation requirements. The registration code for visitors is: CARLETONU-VISITOR
- What is cuScreen?

cuScreen is a web portal that enables employees (including faculty, contract instructors, staff, post-docs, and teaching/research assistants), students, contractors, and visitors to complete vacation attestations and submit a COVID-19 Symptom Reporting Form, as needed.
- How does cuScreen work?

To access cuScreen, visit carleton.ca/covid19/screening and select the appropriate login. Once you log in, cuScreen provides you with a simple questionnaire that produces a colour-coded self-assessment status for you. These questions are based on those provided by the Ontario Office of the Chief Medical Officer of Health.
Based on your responses, cuScreen will provide you with a self-assessment status of red or green. Further instructions on how to proceed are provided by cuScreen based on your status.
In general, a red status means do not attend university premises as planned, and a green status means you may attend university premises as planned.
- What is a COVID-19 self-assessment status?

Generally, a self-assessment status identifies whether a person is at high or low risk of having COVID-19 or having been exposed to COVID-19 based on individual responses to a set of questions developed by public health authorities.
Based on responses to the cuScreen screening self-assessment questions, if an individual is at risk of having contracted or spreading COVID-19, they will be provided with a red self-assessment status.
It is important to note that a cuScreen self-assessment status does not constitute a diagnosis. Please consult your health care provider if you have medical questions.
- What should I do if I receive a red status?

Remaining vigilant and not coming to campus when sick or with symptoms is critically important. If you are feeling unwell, protect your colleagues, classmates and the Carleton community by staying home. You are required to submit a COVID-19 Symptom Reporting Form through cuScreen. Environmental Health and Safety or Health and Wellness Services will be in touch following the report to assess your circumstances and advise you on appropriate isolation timelines.
Vaccination Status
- Do I need a third dose booster to access campus?

Individuals are now able to upload proof of their fourth and subsequent COVID-19 booster doses in cuScreen. In order to report a booster vaccination status, individuals must first report their previous dose information which must be verified by the Carleton verification team.
Individuals must upload a copy of their enhanced vaccine certificate or related document in PDF format. Please do not include any special characters such as a “#” in the name of the file when uploading your proof of vaccination to cuScreen.
Your vaccination document must include your name, name of vaccine(s) and the date(s) you received your dose(s).
Although not required to access campus, we strongly recommend that all members of the Carleton community get their third dose of the COVID-19 vaccine and submit it in cuScreen as soon as possible.
- How do I add subsequent booster doses to cuScreen?

We strongly recommend that all members of the Carleton community get subsequent booster doses of the COVID-19 vaccine as soon as they are eligible.
cuScreen can now accommodate reporting fourth and subsequent booster doses. In order to report your booster vaccination status, your previous dose information must be verified by the Carleton verification team.
- How do I provide proof of vaccination?

To attest to being vaccinated:
- Log into cuScreen
- Click on “Report your latest vaccination status”
- Carefully read each question and answer appropriately
- Upload proof of vaccination (vaccine dose administration receipts or equivalent documents), preferably in PDF format
- Note: your vaccination document must include your name, name of vaccine(s) and the date(s) you received your dose(s)
All Carleton students, faculty and staff are encouraged to attest to their vaccination status and, if vaccinated, upload proof of vaccination and subsequent booster doses in cuScreen as soon as possible.
- What documents are acceptable as proof of vaccination?

Individuals must submit proof of vaccination through the enhanced vaccine certificate or equivalent documents. Please consult your local Ministry of Health or equivalent for more information on how to obtain your proof of vaccination.
Individuals who received their vaccine in Ontario can download their enhanced vaccine certificate through Ontario’s COVID-19 vaccination website.
Individuals who received their vaccine in Quebec can download their proof of vaccine through the Government of Quebec’s website. A French-language version of your proof of vaccination is acceptable.
International students must upload a copy of both the original language proof of vaccination documentation and an officially translated copy. If there is an option to obtain an English-language or French-language version from your government, or if an English-language or French-language version translation is already included on your proof of vaccination, then this will be sufficient.
Vaccination documents must include your name, name of vaccine(s) and the date(s) you received your dose(s).
Any false information or misrepresentation on the mandatory attestation form is a violation of university policy that will be taken very seriously.
- What if my proof of vaccination is not in English?

Carleton University will accept a French-language version of your proof of vaccination.
Individuals who received their vaccine in Quebec can download their proof of vaccine through the Government of Quebec’s website.

- What type of vaccination is acceptable by Carleton?

An individual is fully vaccinated if they have received the full series of a vaccine or a combination of vaccines approved by Health Canada and they received their last dose at least 14 days before the date of their attestation.
For individuals who have received a complete one or two-dose series of a COVID-19 vaccine that is not authorized for use by Health Canada, the individual will be fully vaccinated if:
- they receive one additional dose of an mRNA vaccine; or
- three doses of non-Health Canada vaccine
Individuals are only considered fully vaccinated if they received their last dose at least 14 days before the date of their attestation.
- What if my brand of vaccine isn’t listed in cuScreen?

If the brand of vaccine you received is not listed in cuScreen, you can type in the brand you received and hit the “enter” or “return” key.
For individuals who have received a complete one or two-dose series of a COVID-19 vaccine that is not authorized for use by Health Canada, the individual will be fully vaccinated if:
- they receive one additional dose of an mRNA vaccine; or
- three doses of non-Health Canada vaccine
Individuals are only considered fully vaccinated if they received their last dose at least 14 days before the date of their attestation.
- What file types are acceptable to upload in cuScreen as proof of vaccination?

Carleton University recommends that individuals upload a PDF version of their vaccine dose administration receipt or equivalent document. However, you can upload image files (.png or .jpg). Please do not include a special character such as a “#” in the name of the file.
- Can I update my vaccination attestation at a later date?

Yes. Until you have completed the vaccination attestation to indicate you are fully vaccinated, you can change your vaccination attestation at a later date. This includes submitting proof of full vaccination, which should be done as soon as possible.
- I have already submitted my first dose information. How do I submit my second dose information?

If you have previously submitted your vaccination attestation for your first dose and have since received your second dose, you can update your vaccination attestation.
After logging into cuScreen, click on “Report your latest vaccination status”.
Provided you have met the requirements of being fully vaccinated, you should click “yes” to the first question.
You will be required to submit your first dose information again as well as your second dose information.
- When I view my vaccination attestation form submission under Completed Tasks, my date of birth and health card number is blank. Is the accurate?

Yes. For privacy reasons, Carleton University has not provided date of birth to Thrive Health, Inc. as it is not required to use the platform. Further, Carleton University dose not collect health card information. These fields are left intentionally blank.
- The “Report your latest vaccination status” form has disappeared from Next Steps. Did it go through?

Yes. As soon as you report your latest vaccination status to acknowledge that you have received a full course of a COVID-19 vaccination series, the form will disappear from “Next Steps”. If you click on “Back” and then “COVID-19 Self-Assessment” under “Your Care Network”, your submission will appear under “Completed Tasks”.
- How do I know if my vaccination attestation has been submitted successfully?

As soon as you report your latest vaccination status to acknowledge that you have received a full course of a COVID-19 vaccination series, the form will disappear from “Next Steps”.
To view your submitted vaccination attestation, click on “Care Network” and your cuScreen information. Your submission will appear under “Completed Tasks”.
Symptom Reporting
- What are the current self-isolation guidelines?

For self-isolation guidelines, please visit the symptom reporting webpage.
If you are experiencing symptoms or would like clarification on how long you need to self-isolate, please submit a COVID-19 Symptom Reporting Form through cuScreen. Environmental Health and Safety or Health Services will be in touch following the report to advise on appropriate timelines.
- How do I report symptoms potentially linked to COVID-19?

When you log into cuScreen, click on “Report Symptoms (If Applicable). To complete the COVID-19 Symptom Reporting Form. This form must be completed in the event any employee, student, visitor or contractor has been on campus in the 5 days prior to the onset of symptoms potentially linked to COVID-19.
QR Location Codes
Scanning campus QR location codes with your smartphone camera is no longer required. Please leave all signage regarding QR codes in place should we be required to re-implement this in the future.
Technical
- How do I access cuScreen?

cuScreen is available through web browsers on computers, smartphones, and tablets at carleton.ca/covid19/cuscreen. Individuals with an active MyCarletonOne (MC1) username and password can access cuScreen directly.
Visitors and those who do not have an MC1 account may create a personal account in cuScreen by selecting the appropriate option on the cuScreen landing page.The registration code for visitors is: CARLETONU-VISITOR
- Who should I contact with questions about cuScreen?

- I’m having technical difficulties accessing cuScreen. Who should I contact?

cuScreen is supported by Thrive Health, a leading healthcare technology company. Email support@thrive.health for technical support related to cuScreen.
For technical support related to your MC1 username and password and accessing cuScreen through Carleton’s Single Sign-On, contact the ITS Help Desk.

Privacy and Security
- How is my personal information protected?

Carleton University has partnered with Thrive Health Inc. to provide the cuScreen web portal. As a health care technology company, Thrive Health Inc. has expertise in protecting individual private health information. All data provided or received through the platform is encrypted and your personal information is stored within Canada. Thrive Health Inc. follows data security and privacy standards that align with federal and provincial laws and guidelines such as PHIPA, FIPPA, HIPPA, PIPEDA, PCI, and NIST.
cuScreen has also been assessed by Carleton University’s Privacy Office and Information Security team which confirmed its compliance with applicable privacy standards. For more information, please visit our privacy webpage.
- Can Carleton University access my answers to the health assessment questions?

No. The university cannot access the answers you submit to the cuScreen self-assessment questions and does not require your specific answers for the paper-based self-assessment and location log.
For example, if you answered “yes” to the question of whether you have been out of Canada in the last 14 days and received a red self-assessment status, the university will not have access to your answers and will not know why your self-assessment status is red.
- Who at Carleton will have access to my COVID-19 screening status?

For cuScreen, this will depend on whether you are a faculty, staff, or librarian, or a student:
Faculty members, librarians, staff, contractors and visitors
Individual employee self-assessment status (green status and red status) will be shared with:
- The appropriate human resources departments/supervisors;
- Directors, deans, and senior-level management according to reporting relationships as documented in Human Resources;
- Environmental Health and Safety (EHS) to provide follow-up support (as appropriate, and for potential contact tracing); and
- Health and Counselling Services (as appropriate) for follow-up reporting (potentially including contact tracing).
Students
Individual student self-assessment status (green status and red status) will be shared with:
- Registrar’s Office/Faculty of Graduate and Postdoctoral Affairs (as appropriate) to potentially provide follow-up academic and registrarial supports;
- Residence Administrators to provide residence-related follow-up and support; and
- Health and Counselling Services (as appropriate) for follow-up reporting (potentially including contact tracing).
In all cases, you may be asked by a cuScreen Ambassador or another Carleton University employee to show your COVID-19 screening self-assessment status prior to entering a university building or visiting an office.
- What are the requirements for using a public computer?

If using a public computer, please ensure you log out of cuScreen after completing your COVID-19 screening self-assessment and close your browser as you would any other application.
Employees, Contractors and Affiliates
Including faculty, contract instructors, staff, post-docs, teaching/research assistants, C&W, Aramark, etc.
- What happens if I get a red self-assessment status?

Students
- Will I be expected to show my self-assessment status before entering a class?

No, self-assessment are no longer required.

- I received a red status. Where do I go to get help?

Everyone with symptoms of COVID-19 should stay home, not attend campus, classes or work, and self-isolate. Please complete the online symptom reporting process.
- What should I do if I will be missing class time for COVID-related reasons?

Students are encouraged to connect directly with their instructors to discuss required accommodations arising from COVID-19.
- Will I be denied access to Carleton University buildings, classes, and/or exams if I receive a red status?

If you are feeling unwell, protect your colleagues, classmates and the Carleton community by staying home. You are required to submit a COVID-19 Symptom Reporting Form through cuScreen. Environmental Health and Safety or Health and Wellness Services will be in touch following the report to assess your circumstances and advise you on appropriate isolation timelines.
If you live in residence, please stay in your room and contact residence staff. Following the advice of public health authorities will help to reduce community spread.
