Carleton University highly recommends all individuals use the cuScreen system to help ensure the health and safety of the Carleton community. cuScreen can be used on a mobile device, tablet or laptop.

In all cases, your vaccination attestation must be completed through cuScreen.

If you are unable to access cuScreen to complete your COVID-19 Screening Self-Assessment, you can instead use the paper-based self-assessment and location log to document your screening self-assessment results. If using this paper log, you must present proof of vaccination and photo ID. If you have an approved exemption, you must also present a negative rapid test result no older than 72 hours.

You must complete a paper-based self-assessment and location log each day you come to campus and you must present proof of vaccination each time you enter a building.

You are required to keep the paper-based self-assessment log on you throughout the day and submit it via one of the cuScreen drop-boxes located in the lobby of Robertson Hall or at the Welcome Centre in Alumni Hall, or to a cuScreen Ambassador at the end of each day.
