Under Orders from the Government of Ontario and guidance from Public Health Ontario and Ottawa Public Health, Carleton University is required to implement COVID-19 screening for all employees (including faculty, contract instructors, staff and teaching/research assistants), students, and visitors. The short COVID-19 Screening Form must be completed each day you plan to enter a Carleton University building.
Please note: the screening form for employees and students does not work on Internet Explorer.
All employees (including faculty, contract instructors, staff and teaching/research assistants) must complete the short COVID-19 Screening Form prior to coming to campus and entering a building, including a restricted access building. This form must be completed every day you come to campus.
Employees are expected to continue to work remotely if their duties can be performed remotely. If you are required to be on campus, and in addition to completing the COVID-19 screening form, you must follow the existing building access protocols and complete the required training on cuLearn.
If entering a publicly accessible building, employees will need to show the email confirmation (either on their mobile device or a printed copy) to the attendant or complete a new screening form before being allowed to enter the building.
All students must complete the short COVID-19 Screening Form prior to coming to campus and entering a building, including publicly accessible buildings. This form must be completed every day you come to campus.
Students living off-campus are strongly advised not to come to campus unless it is essential, or you have made the appropriate arrangements to access limited facilities. If you do come to campus, and you are accessing a building that is not publicly accessible you must follow the existing access protocols and complete the training on cuLearn.
Students living in residence must continue to complete the COVID-19 self-assessment daily and follow the Department of Housing and Residence Life’s symptom reporting process. The COVID-19 screening form must be completed if you are accessing a building that is not your residence building or Residence Commons.
Contractors must follow the existing screening practices already established.
Visitors accessing a publicly accessible building must complete the COVID-19 Screening at the entrance to the building prior to being allowed to enter. Please respect and follow all guidance from the attendant.
The Department of Recreation and Athletics has implemented a separate screening process. Please follow the screening process as required before entering the athletics facilities.
