The Feedback activity allows you create customizable surveys to collect feedback from students. Unlike the Quiz tool, the questions are non-graded and can be anonymous. The feedback activity is a great resource for course evaluations, such as midterm feedback surveys or peer evaluations, but it can also be used to collect feedback on any course topic.

Creating a Feedback Activity

  1. Log in to cuLearn using your MyCarletonOne credentials.
  2. Click on your course link.
  3. Go to the topic/section in which you’d like to add the Feedback survey.
  4. In the Add an Activity drop-down menu, select Feedback.
    NOTE: If you are unable to see the Add an Activity menu, your course editing may be turned off. To turn on course editing, click the settings gear on the top-right side of your course page, and select Turn editing on from the drop-down menu.
  5. On the Adding a New Feedback page, you have the following options for customizing your survey:
    1. In the General Section, enter the following:
      • Name – Give your Feedback activity a title.
      • Description – This text will be displayed to users before they enter the feedback activity.
    2. In the Availability section – Enable Allow answers from and Allow answers to dates to set a time period during which the Feedback will be open/available for responses.
    3. In the Question and Submission section, you have the following settings options:
      • Recording user names –  Responses are anonymous by default. If you don’t want responses to be anonymous, select User’s name will be logged and shown with answers in the drop-down menu.
      • Allow multiple submissions – If enabled for anonymous surveys, users can submit feedback an unlimited number of times.
      • Enable notification of submissions – If enabled, teachers will receive notification of feedback submissions.
      • Auto number questions – Enables or disables automated numbers for each question.
    4. In the After Submission section, you have the following settings options:
      • Show analysis page – Select whether you want your students to see an analysis page providing an overview of the feedback results once they have submitted their feedback.
      • Completion message – You have the option to enter a completion message that will be displayed to student once they have submitted their feedback (ex/ “Thank you for providing your feedback”)
      • Link to next activity – You can enter a URL in this field if you want to direct your students to separate webpage once they click “Continue” after submitting their feedback.
  6. When you are finished configuring your settings, click Save and Display.

You will now see a link to your feedback activity displayed on your course page (see example below)

Adding Questions

Once you have created your Feedback survey and configured the activity settings, you add your survey questions.

To add questions:

  1. Log in to cuLearn using your MyCarletonOne credentials.
  2. Click on your course link.
  3. Click on the link to your Feedback activity on your main course page.
  4. Click the Edit Questions tab at the top of the Feedback page
  5. Select a question type from the Choose… drop-down menu.
  6. Fill in the question details.
  7. Click Save question.

Once you have added a question, you can view/edit it under the “Edit questions” tab.

Viewing Feedback Results

  1. Log in to cuLearn using your MyCarletonOne credentials.
  2. Click on your course link.
  3. Click on the link to your Feedback activity on your main course page.
  4. The results of the Feedback activity can be viewed in the following ways:
    1. Click the Analysis tab at the top to see a shows a graphical summary of the results of each question. The analysis can be Exported to Excel.
    2. Click the Show responses tab at the top see and if needed delete individual responses. The response data can be downloaded in various formats.

Adding a Pre-made Midterm Feedback Survey Template

Adding a midterm feedback survey is a great way to learn what is and is not working in your course.  By asking students what they like, what they don’t like, and what could be improved in your course, you can act upon the students’ suggestions and make any necessary changes before the semester is over. Learn more about the benefits on our Feedback on Teaching page, which includes some commonly used questions.

We also have a feedback survey template we encourage you to use containing the most used basic questions.

To use the template:

  1. Click on the Templates tab
  2. Under the heading ‘Use a template’, select ‘Midterm Feedback Evaluation’ from the drop-down menu.


3. You will be brought to a new screen asking if you’re sure you’d like to use the template.  If you are happy with the three generic feedback questions, select, “Delete old items” and click ‘Save changes’.    

TIP: You can add new questions by clicking on the drop-down menu, edit the existing questions by clicking on the Edit link beside each question, or simply return to your main course page (by clicking on the name of your course).
