Supervisors have a responsibility to maintain the safety and well-being of those that report to them. This includes reporting and investigating injuries and incidents as well as taking action to prevent them.

Previously, supervisors were required to fill out the “Supervisor’s Injury/ Illness Reporting and Investigation Form” and then determine if a WSIB Form 7 was required based on the extent of the injury.
