Applicants to the Enriched Support Program are asked to provide two academic references (from two teachers, or a teacher and a guidance counsellor). Please note: the forms below are intended for teachers and guidance counselors.
For applicants who have been away from studies for 3 or more years and who are providing community or workplace references, please consult our Reference Guidelines instead.

Please be sure to provide a valid school board or education institution email address when completing a Reference Form.
PDF Form for Download
Download our Reference form as a pdf file and email it to email@example.com.
Please email the completed form from your school board/college/university email (as applicable) address.
Note that the form will not save if it is completed in the web browser instead of Adobe Reader.
Steps to make sure the .pdf form saves correctly:
- Download our Reference form as a pdf file.
- Open it in Adobe Reader. (Do not complete the form in a browser window; it will not save correctly.)
- Fill out the application.
- Save it.
- Email the saved document from your professional email account to firstname.lastname@example.org.
