Starting June 2, 2021, Carleton’s Health and Counselling Services will begin administering first-dose mRNA vaccines, subject to availability, for eligible people ages 18 or older in 2021. In order to better facilitate appointment bookings and to help determine need, members of the Carleton community are asked to pre-register for their vaccine as soon as possible.
In order to pre-register for an appointment, you must be:
- Aged 18 or older in 2021
- A Carleton employee, student or approved contractor
- Eligible for the first vaccine dose only
After pre-registering online, a Patient Coordinator from Health and Counselling Services will email you within 24-48 hours to confirm the date and time of your vaccine.

Personal information, including personal health information, collected through this form will be used and disclosed by Carleton University in accordance with Part III of the Freedom of Information and Protection of Privacy Act, and Part IV of the Personal Health Information Protection Act. Our authority to collect your personal information is derived from the following legislation;
- Carleton University Act;
- Health Protection and Promotion Act;
- Occupational Health and Safety Act; and,
- Reopening Ontario Act.
The purpose of this processing is;
- To register you for a COVID-19 vaccination appointment at Health and Counselling Services.
- To create and/or update your patient health record at Health and Counselling Services.
The information is stored on servers maintained by Carleton University ITS. Your personal information is protected through a variety of controls, such as card access to servers, multi-factor authentication, and enforcement of data protection policies.
