If you are Carleton student, you are covered by your student association plan. If you are an undergraduate student, please visit the CUSA page for more details. If you are a graduate student, please visit the GSA page for more details.

Contract instructor benefits are provided by CUPE4600. Please contact your union for more information about your coverage.

If you are a student and employee, you need to notify the Registrar’s Office and Human Resources of the change. Please provide both offices a copy of your citizenship document.

If you are a Carleton employee, you only need to notify Human Resources. Please provide a copy of your citizenship document by email in the form of a secure PDF.

If you are a Carleton University student and employee, you need to notify the Registrar’s Office and Human Resources. Contact the Registrar’s Office and provide  them proof of your Permanent Residency. You must also submit your new Social Insurance Number document to Human Resources by email in the form of a secure PDF.

If you are a Carleton University employee, you will only need to notify Human Resources. Please submit proof of your PR status and new Social Insurance Number document to Human Resources by email in the form of a secure PDF.

To either apply, or obtain confirmation of a your SIN, gather all of the required original documents to prove your identity and status in Canada and take them to the nearest Service Canada point of service.

Yes, every person working in Canada needs a valid SIN to work. You can find more information about applying for a SIN at Service Canada.

It is the worker’s responsibility to report any workplace hazards to their immediate supervisors. A hazard is defined as any practice, behaviour, condition, thing, situation, or a combination of these things, having the potential to cause injury or illness to a person or damage to property or equipment. For more information, please consult Carleton’s Hazard Reporting Policy.

In a workplace incident, no injury or illness occurs. An incident is defined as an event that could have resulted in an illness or injury if a slightly different set of circumstances had transpired. Examples of this can include: fires, explosions, spills, etc. Incidents like workplace injuries still need to be reported to the workplace supervisor. By definition, a workplace injury is classified as any illness, injury, or acute psychological trauma caused by physical, chemical, or biological hazards in the workplace. All workplace injuries or illnesses must be immediately reported to the workplace supervisor. For more information, please consult Hazard Identification and Reporting.

If your employee status changed this year, you will receive two T4’s (one for each reporting category). Be sure to select all statements from the Tax Form drop-down menu in Carleton Central.

Examples of status change:

• Casual employee appointed as a salaried employee
• Member of CUPE 2424 / Admin Tech-Exempt appointed to Administrative Management
• Contract instructor appointed to a term/continuing faculty appointment

Employee contributions to extended healthcare and dental plans.

Box 40 reports taxable benefits and may include the following:

• Group life premiums (basic coverage) paid by the employer on behalf of employees. This includes sales tax
• Miscellaneous taxable benefits

Amounts reported in box 40 are also included in box 14 (total income).
