New Employee Documentation Forms

If this is your first employment assignment at Carleton University, fill out the forms below and submit a completed documentation package to human resources within a week of your start date.

A completed documentation package includes the following:

Temporary residents must also submit:

  • Letter of Confirmation of Social Insurance Number (SIN)
  • Photocopy of valid immigration document indicating eligibility to work (Work Permit or Study Permit)

Any delays in receiving the forms and verification will result in a delay in pay.
