Some dental treatments require pre-approval before they can begin. This is called Predetermination of Benefits.<\/p>\n\n\n\n<p>Many dentists will submit claims directly to <a href=\"https:\/\/my.canadalife.com\/climsMyLogin\">Canada Life<\/a>.&nbsp;You need to provide dentist with your employee ID (member ID) and the dental policy number (51801)<\/p>\n\n\n\n<h3 id=\"submit-your-claim-online\" class=\"wp-block-heading\">Submit your Claim Online<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Some dentists will submit claims to Canada Life but require payment for services.<\/li>\n\n\n\n<li>Submit your payment as a claim through <a href=\"https:\/\/my.canadalife.com\/climsMyLogin\">Canada Life<\/a>. Canada Life will send the reimbursement directly to you<\/li>\n<\/ul>\n\n\n\n<h3 id=\"submit-your-claim-via-mail\" class=\"wp-block-heading\">Submit your Claim via Mail<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If your dentist does not use electronic submission they should fill out the \u201cDentist\u201d section on the Standard <a href=\"https:\/\/carleton.ca\/hr\/wp-content\/uploads\/sites\/16\/Canada-Life-Dental-Claim-Form.pdf\">Dental Claim Form<\/a>.<\/li>\n\n\n\n<li>Complete the remainder of the form and submit to Canada Life<\/li>\n\n\n\n<li>Claims must be submitted within 6 months of the date of service<\/li>\n\n\n\n<li><strong>Note:<\/strong> Do not sign the \u201cassignment to dentist\u201d section if you have paid the dentist<\/li>\n<\/ul>\n\n\n\n<h2 id=\"drug-claims\" class=\"wp-block-heading\">Drug Claims<\/h2>\n\n\n\n<p>Prescription drugs may be claimed either at the time of purchase by using the Pay Direct Drug Card or submitted later to <a href=\"https:\/\/my.canadalife.com\/climsMyLogin\">Canada Life<\/a> using a Healthcare Expenses Statement form.<\/p>\n\n\n\n<h3 id=\"prescription-drug-pre-authorization\" class=\"wp-block-heading\">Prescription Drug Pre-Authorization<\/h3>\n\n\n\n<p>Although Canada Life is the insurance company, many prescription drugs require prior-authorization. In our plan the prior authorization is administered through the FACET program. They can be reached at <a href=\"https:\/\/www.facetprogram.ca\/en\/\" target=\"_blank\" rel=\"noreferrer noopener\">Home|FACET<\/a>.<\/p>\n\n\n\n<p>It is recommended that before an employee purchases any prescription drug that they use the Drug Search feature of their Canada Life account to confirm if prior authorization is required. Cost of the medication only, and not the doctor\u2019s fee will be eligible for reimbursement<\/li>\n<\/ul>\n\n\n\n<h2 id=\"vision-care-claims\" class=\"wp-block-heading\">Vision Care Claims<\/h2>\n\n\n\n<h3 id=\"claims-for-eye-examinations\" class=\"wp-block-heading\">Claims for Eye Examinations<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Submit claims using the <a href=\"https:\/\/carleton.ca\/hr\/wp-content\/uploads\/sites\/16\/Canada-Life-Healthcare-Expense-Form.pdf\">Healthcare Expenses Statement<\/a> or online using <a href=\"https:\/\/my.canadalife.com\/climsMyLogin\">Canada Life<\/a><\/li>\n<\/ul>\n\n\n\n<h3 id=\"claims-for-glasses-and-contacts\" class=\"wp-block-heading\">Claims for Glasses and Contacts<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Submit claims for glasses and contacts using the <a href=\"https:\/\/carleton.ca\/hr\/wp-content\/uploads\/sites\/16\/Canada-Life-Healthcare-Expense-Form.pdf\">Healthcare Expenses Statement<\/a><\/li>\n\n\n\n<li>Obtain an original itemized paid receipt which includes: the vision prescription, a breakdown of charges for lenses and frames, and the date the glasses were picked up<\/li>\n\n\n\n<li>Claims must be submitted within 18 months of the date of service<\/li>\n\n\n\n<li><strong>Note<\/strong>: If you want to submit online using <a href=\"https:\/\/my.canadalife.com\/climsMyLogin\" target=\"_blank\" rel=\"noreferrer noopener\">Canada Life<\/a>, you may do so, but ensure you have Section 4 of the Visioncare Claim Form filled out for your records<\/li>\n<\/ul>\n\n\n\n<h2 id=\"extended-health-care-claims\"
