The Paul Menton Centre (PMC) has developed specific documentation requirements and forms for various types of disabilities. The following list provides links to documentation forms that include information required by the PMC to formally document a disability. For learning disabilities, a psychoeducational assessment report is necessary and the link below outlines specific assessment requirements. Please also review detailed information about certain disability categories.
- Attention Deficit/Hyperactivity Disorder (ADHD)
- Autism Spectrum Disorder
- Chronic Medical Disabilities
- Head Trauma / Acquired Brain Injury
- Hearing Impairments
- Learning Disabilities
- Medical Cannabis
- Mobility Disabilities
- Psychiatric / Mental Health Disabilities
- Service Animal
- Visual Impairments
Every new student registering with the PMC will be asked to complete the PMC Assessment of Functional Impairment (AFI). The AFI will be emailed to you and is used to evaluate the nature and the degree of functional impairment due to disability, in order to inform PMC support services.
NOTE: if your documentation is in French and/or if French is your preferred language, please inform the desk at the time of your registration with PMC. This will help us to ensure that you are booked with a PMC Coordinator who will be able to best meet your needs.
