Pursuant to Orders from the Government of Ontario and guidance from Public Health Ontario and Ottawa Public Health, all visitors accessing the Carleton University campus are required to complete a COVID-19 Screening process. As a valued supplier of goods and services for Carleton University, we ask you to ensure that any of your employees, suppliers or contractors accessing campus follow the daily COVID-19 screening self-assessment process prior to accessing campus, and check in at campus locations if that individual intends to stay in a location for more than 15 minutes. To do so, suppliers and contractors must create a visitor account following the instructions outlined on the cuScreen information site, unless a specific registration code has been provided to your organization by the university. This requirement extends to any delivery service or employee making deliveries on your behalf.
Furthermore, as a result of the recent strong recommendation of the Council of Ontario Medical Officers of Health (COMOH), which represents all 34 public health units across the Province, Carleton University along with all post-secondary institutions in Ontario, will be requiring mandatory vaccination against COVID-19 for all individuals (students, staff, faculty, contractors, and visitors) involved in any in-person Carleton activities both on- and off-campus.
All individuals who are coming onto campus must attest to receiving their first dose vaccine by Sept. 8, 2021, and their second dose no later than Oct. 15, 2021. This includes all suppliers and contractors working on Carleton’s campus or accessing Carleton facilities.
Proof of vaccination must be provided through cuScreen by the deadlines referenced above. More information on how to upload and provide proof of vaccination will be communicated at a later date. Any false information or misrepresentation on the mandatory attestation form is a violation of university policy that will be taken very seriously.
Any individual who is not fully vaccinated will be required to submit rapid testing results every 72 hours, and prior to arriving on campus.

Note: Reporting vaccination status is not required to access campus prior to Sept. 8, 2021 and will be communicated when it is available.
