Congratulations on your Professional Achievement Award! In accordance with Article 42:4: Professional Achievement Awards, of the Collective Agreement with CUASA, you may choose to receive your award of $10,000 as a lump sum payment or as a professional expense reimbursement. Details about each option, including income tax implications, are outlined below.

  • A) Payment Options

    Notes:
    Option 1: Lump sum payment A cheque for a lump sum payment in the gross amount of $10,000 will be taxed as salary income.
    Option 2: Professional Expense Reimbursement A Professional Expense Reimbursement in the amount of $10,000 for the next academic year in accordance with Article 40.13.
    Receipts are required for reimbursement. There are no tax implications for this option.
  • B) Publication

  • Please submit a short (approximately 75 words) paragraph via email (to provost@carleton.ca) outlining your interests and your application for submission in Carleton Now and on our websites.
  • C) Awardee Information

  • Kindly confirm your acceptance of this Award as well as your preferred payment method by submitting this completed form by January 31.
