There may be occasions on which you choose to authorize the release of personal or academic information to a third party specific to the information managed by the Registrar’s Office. On these occasions, a completed Third Party Release of Information Form must be signed authorizing such a release. This form is kept in the student file and is referred to before releasing any information to a third party. Please note: Upon submission, the Third Party Release form must be accompanied by your photo ID, with both the photo and signature showing.
