This form must be completed by a university employee.<br>Email the&nbsp;documentation that indicates the insurance requirements to&nbsp;<a href=\"mailto:risk@carleton.ca\">risk@carleton.ca<\/a><br><br><strong>Employees<\/strong><br>If you are required to provide proof of insurance prior to accessing another company or institutions&#8217; premises\/lands for university purposes,&nbsp;complete the below process.<br>Complete the online form&nbsp;(ADD LINK).&nbsp;This form must be completed by a university employee<br>Email the&nbsp;documentation that indicates the insurance requirements to&nbsp;<a href=\"mailto:risk@carleton.ca\">risk@carleton.ca<\/a><br><br><strong>Students<\/strong><br>If you are required to provide proof of insurance prior to accessing another company or institutions&#8217; premises\/lands for university purposes (i.e.&nbsp;&nbsp;graduate student research, course work, etc),&nbsp;complete the&nbsp;below process.<br>Contact your department and ask that they&nbsp;complete the online form&nbsp;(ADD LINK).
