All requests for copies of Campus Safety Services Occurrence reports are forwarded to the Freedom of Information and Protection of Privacy Act (FIPPA) office for review. If you would like to request a copy of a report, please complete a FIPPA Access request form.

Please include the following in the description section:
- Date of Birth and /or Employee ID or Student ID number.
- Campus Safety Services occurrence report number, if known ( example: 16-1234)
- Date of occurrence on campus
- Brief description of occurrence
Forward the completed document to Carleton University Privacy Office, along with application fee as outlined on form.
The request will be reviewed once payment is received.
