3. Individual Driving Assessments: Occupational Therapists (OTs) whose practice involves driver assessment (there will likely be a cost with this service).

Use the Find an OT service on the www.caot.ca website:

Find an Occupational Therapist

Put in the city and then select Driver assessment & rehabilitation and you will get a list of OTs (who offer driver assessment services) and their contact information.
