Complete the reporting form below to tell us about the human rights harassment or discrimination you experienced. Reminder: this is not considered a formal complaint, should you wish to submit a formal human rights complaint please contact the appropriate Intake Office.

Anonymous reports under the Human Rights Policy may not be investigated, at the discretion of the University, but the information will be reviewed and any findings will be used as an important input in creating an inclusive campus culture.
