Please ensure that you are submitting this request from a location outside of Ottawa and beyond the 160km radius. Falsely claiming a different city than your actual location will lead to the denial of your request upon verification or finding out.

If you are approved to write your exam online, please make sure you take the exam in the exact city and country you mentioned in your application. If it is found that you took the exam in a different location, your application will be canceled, and an instructional offense will be reported/sent to the dean for taking the exam under false pretenses.

I acknowledge that a late fee may be applied to my account should I submit my request after the application deadline.

I acknowledge that if I cancel my request within 7 days of the scheduled exam date, a cancellation fee may be imposed on my student account. Additionally, if no cancellation request is made and I fail to attend the exam, the full distance fee will be charged to my student's account.

I acknowledge that the e-Proctoring solution CoMaS could be used for the purpose of invigilating my distance exam.

I acknowledge that I may need to travel to a nearby exam site if an in-person exam is required by the course instructor.

I acknowledge that if a suitable exam location cannot be arranged at my preferred location, I may need to travel to another location within 160 km of my area.

I acknowledge that my exam time or date may be changed if an exam center is not available at the originally scheduled time, with proper notice provided to me.

I acknowledge that if I am writing my exam at a center, and I have been approved and given instructions to book my spot, I must complete the booking in a timely manner to avoid cancellation of my exam.
