Practicum Forms

Practicum Request Form: This form is to be filled out by the student when you are interested in a practicum placement for an upcoming term and is for current Curatorial Studies diploma students only.

The three following online forms must be completed and submitted before an override for registration in any practicum course is processed:

• Practicum Description/Agreement Form: This form is to be filled out by the prospective practicum supervisor from the partnering institution. It is required ahead of any practicum placement and will be forwarded to our Practicum Coordinator and the student.
• Letter from Placement Employer: This form is to be filled out by the prospective practicum supervisor from the partnering institution (the employer) or by an official authority from the partnering institution. It is required ahead of any practicum placement and will be forwarded to our Practicum Coordinator and ICSLAC’s administration.
• Student Declaration of Understanding and Agreement: This form is to be filled out by the student. It is required ahead of any practicum placement and will be forwarded to our Practicum Coordinator and ICSLAC’s administrator.
