Graduate Forms

Please download and fill out the appropriate form and forward it to the graduate administrator of the department. These files are available in PDF format. You will require Adobe Reader® to view PDF forms.

Academic/Registration Change Form Request

The Academic/Registration Change Form must be filled and approved by your supervisor (or advisor) and the Department. Types of requests include:

Change Program Form

Any requests to change your current registration must be made through the Change Program Form. This may include changes from a Master’s program to the doctoral program, or changes within the Master’s program, such as switching from a course work M.Eng. program to project M.Eng or from M.Eng. to M.ASc.

Course Approval Request form

Courses taken outside the Civil or Environmental institute require approval by the advisor/supervisor, the Civil or Environmental director/associate director of the institutes, and the departmental Associate Chair. Please ensure that you submit your Course Approval Form prior to the beginning of the term.

Course Request Form for Special Students

This Course Request Form is for Special Students wanting to register for courses offered within the Civil and Environmental Engineering Department.

Course overload request form

The Course Overload Form is for students who have full-time status wishing to register in more than the allowed 2 credits (i.e. 4 courses) for the term. This form does not apply for part-time graduate students as the requirement for part-time status is to only take 1.25 credits per term. Please also note that for students with full-time status, the Thesis/Project credits are counted as one course within this limit.

Directed Studies

A Directed Studies Agreement must be signed by the student, instructor, and the Chair of the Department prior to the last day for registration in order for a directed studies course to be counted toward the requirements of the program.

Extension Request Form

The normal times for completion are provided in the Graduate Calendar; a Master’s program is two years, and a Doctoral program is six years. If you require more time, you should make a request using the Extension Request Form.

Request to Audit a Course

Auditing a course means that you are formally registered in a course and are permitted to attend lectures but you are not required to submit coursework or write final exams.
