Placement Agreement Template
Introduction
As part of the educational experience, students have opportunities for experiential learning within community settings. These may include – but are not limited to – practicums, course-based projects and service learning. Carleton University wants to ensure that our students have a safe and enjoyable experience while participating in unpaid placements.
Purpose of the Template
Student placement agreements are developed between Carleton University and various community organizations when students are scheduled to complete an unpaid placement (online or in-person) as part of their course and/or program of study. These agreements are required by the university before a student can begin working with the community organization, as they provide the necessary coverage for all parties involved.
Using the Template
Before using the template, please check if a placement agreement already exists between Carleton University and the specific community organization. If so, review the existing agreement as it may already provide sufficient coverage for the placement to occur. If a new agreement is required, you may proceed with using the Student Placement Agreement Template.
Should the placement organization prefer to use their own template for this agreement, please contact the Office of Academic Programs and Strategic Initiatives (APSI) for support throughout this process.
The Student Placement Agreement Template has been designed for ease of use. The document may be downloaded by Carleton staff and/or faculty members and reviewed by the community organization. Should the community organization wish to make edits to the text in this template, please contact APSI to begin these discussions.
Agreement Signatures
If both parties agree to the terms outlined in Carleton’s student placement agreement, the document is to be completed and signed by the Provost and Vice-President (Academic) Carleton University (facilitated through APSI) and the community organization.
While this agreement can be used at the faculty level, we strongly encourage the development of university-wide agreements to provide coverage for all Carleton students who may engage in future placements with the community organization.

Student Attestation Form
Introduction
As part of their educational experience, students have opportunities for experiential learning within community settings. These may include – but are not limited to – practicums, course-based projects and service learning. Carleton University wants to ensure that our students have a safe and enjoyable experience while participating in unpaid placements.
There are several requirements that students must complete before they may begin their experiential learning experience. These may include – but are not limited to – screening and vaccinations, mandatory trainings, and the completion of a formal Police Record Check. Most, if not all of these, require the handling of students’ personal information.
Freedom of Information and Protection of Privacy
The university is responsible for the security of students’ personal information under the Freedom of Information and Protection of Privacy Act (FIPPA). It is important that our practices in dealing with students are in line with the requirements of the act. The Student Attestation Form was created to protect the personal information of students.
Purpose of the Student Attestation Form
A form has been developed where:
- Placement requirements are communicated clearly to students by Carleton University and the community organization.
- Students attest to the completion of their placement requirements.
With the development of this tool, Carleton staff and faculty members will not encounter student personal information as it relates to student placement experiences.
Using the Template
Instructions on using the form are listed below:
- Representatives from Carleton University and the placement organization work together to ensure all placement requirements are indicated in the Student Attestation Form.
- Once all requirements are clearly listed, the form is delivered via email to the student for completion.
- The student works to complete all listed requirements.
- Once all requirements are executed by the student, the student completes and signs the form.
- Once signed, the form is returned to Carleton University for review and signature.
- Carleton University then informs the placement organization of the student’s completion of all placement requirements.
- Carleton University then sends a copy of the completed form to the placement organization for review and approval.
If there are any issues (e.g. the student cannot complete one or more of the listed requirements), they must inform Carleton and the placement organization immediately.
Agreement Signatures
The completion of this form may require the signature of other individuals, such as the student’s primary care (family) physician, in addition to Carleton University and the placement organization.
