What happened to the WSIB Form 7? Are injuries no longer reported to WSIB?
There have been no changes to the reporting process with the Workplace Safety Insurance Board (WSIB). The WSIB is still notified of injuries involving lost time and still receive Form 7s from Carleton University.
However, supervisors no longer need to fill out a separate Form 7 when submitting an injury report online. If the nature of the injury requires the WSIB to be notified, a Form 7 will be generated from the information included in the CU WorkSafe injury report and submitted to WSIB, as before.

Only Good Catch Reports can be submitted anonymously. To do so, please enter CU WorkSafe via the Guest Portal and select “Good Catch Report”.
Any Good Catch Reports submitted while logged in will not be anonymous.

For our purposes, first aid is not considered healthcare. To be included in the healthcare classification, treatment must involve the care of a health care professional (e.g. doctor, nurse, paramedic, etc..).
Visiting a hospital, walk-in clinic, family doctor, etc.. are examples of instances where “Health Care” should be selected in the report form.
If you are unsure of which box to check, include a description of the care received in the notes section so the report can be amended if appropriate.
