Consent for Use of Personal Information (Linguavision)

Please complete this consent form in advance of the contest.

I authorize the:	School of Linguistics & Language Studies
	Office / Program / Individual
To use the designated photos, audio & video recordings:	I am performing in the Linguavision Song Competition. I authorize organizers to record (video & audio) and take photos of my performance. I permit the use of the resulting photos and videos of the event and to use them in future promotional materials including but not limited to online video, photos, posters, brochure art, and audio recordings. I consent to the use of my name and the language I am studying. I am also sending event organizers a bio and photograph. I consent to the use of these materials on the event website, on posters, on social media, and in any and all other promotional materials.
	Listing of photos and audio or video to be disclosed.
Recorded:	Event recorded 31 March, 2026; bios & headshots submitted in February 2026.
	Date photos and audio or video recordings made.
Purpose:	To allow organizers to collect photos and audio/video recordings of the event and use these photos and recordings create a commemorative video, commemorate the event, create promotional materials for Linguavision, and materials to promote the School’s language courses.
	State specific purpose of information release.
For the period:	Indefinitely
	State date range for which permission will exist.

Consent/Release:

I do hereby release and forever discharge Carleton University, its officers, employees, representatives, and assigns from any and all demands, cause of actions and/or judgments of whatsoever nature of character, past or future, known or unknown, whether in contract or in tort, whether for personal injuries, property damage, payments, fees, expenses, or damages of any kind or nature, and whether arising from common law or statute, arising out of, in any way, the use of my photographs, text, audio and video recordings for any purpose. This agreement does not include, or supersede other personal information agreements signed by me at Carleton University and concerning my personal information.
