COVID-19 Self-Screening for FMP Staff

The Ontario Ministry of Health is now requiring Ontario workplaces to implement COVID-19 screening before a worker enters the workplace at the beginning of their day or shift.

Facilities Management and Planning is using the Ottawa Public Health’s screening  tool provided on their website to meet these requirements.  Instructions in the tool should be followed.

Individuals who do not pass screening are advised that they must not enter the workplace, should self-isolate and must notify their manager. When an individual has passed the screening questions, they are allowed to enter the workplace.
