Documentation forms – one for each disability category – have been developed by the PMC to gather relevant information that your PMC coordinator needs to determine appropriate accommodations and support services during your academic studies at Carleton.

These forms may be used in-place-of and/or in conjunction-with existing or past documentation. Please review the documentation requirements for the relevant category, and the respective documentation form(s) below.

If necessary, ask your regulated health care practitioner to complete it prior to your intake appointment.

Acquired Brian Injury (ABI)

Attention Deficit/Hyperactivity Disorder (ADHD)

  • A copy of your latest comprehensive psychoeducational assessment, completed by a registered psychologist or psychological associate. An assessment completed within the last 5 years, or at the age of 18 years or older, is preferred. OR…
  • A PMC Documentation of ADHD Form completed by a regulated health care practitioner.

Autism Spectrum Disorders

  • A psycho-educational assessment (PEA), completed within the last 5 years or performed using adult-normed tests. OR…
  • A PMC ASD documentation Form completed by a regulated health care practitioner who can confirm diagnosis and provide information on current functional limitations.

Chronic Medical Conditions

  • A PMC Documentation of Medical Disability Form completed by a regulated health care professional is preferred.
  • If it is not possible to get this form completed in time for your intake appointment, we will accept other medical documents that confirm that nature of your disability and functional impairments relevant to the university setting as interim documentation.

Deaf and Hard of Hearing

Learning Disabilities (LD)

A recent Psycho-Educational Assessment (PEA) completed by a registered psychologist or psychological associate within the last 5 years or at the age of 18 years or older is preferred. Please visit our webpage for more information about LD and documentation requirements.

If it is not possible to provide a current PEA, we will accept the following documentation for interim accommodations and support until an updated assessment is completed:

  • Psycho-educational Assessment 6 years or older.
  • An Individual Education Plan (I.E.P) that references a learning disability and previous LD assessment.

Mobility/Physical Disabilities

Mental Health Disabilities

  • A PMC Documentation of Mental Health Disability Form completed by a regulated health care professional who has knowledge of the student’s history and is licensed to diagnose and treat mental health disorders. OR…
  • A copy of a recent psychological assessment report completed by a registered psychologist or psychological associate.
