How to fill out Questionnaires
Log in to MySuccess – Student Login
Click on School of Social Work
Click on “current experiences.”All the experiences are listed on the right-hand side of the page.
Click the current step, “Application,” in this case, and click “Answer Questionnaire.”
- Fill in the required details. All the questions with * are mandatory
- You can only choose one option. When selecting “I require accommodation,” an additional box appears for students to describe their specific needs.
Choose areas of Interest and agencies of interest
Please upload your resume in MS Word format.
To complete the Workplace Placement Proposal form and the Distance Placement form,
- Go back to the workflow,
- Click on Back to Enrollment.
- Choose the appropriate form and click “Answer Questionnaire.”
