Please complete this form even if your course has no formally scheduled examination. Please fill out a form for each exam unless you teach the same course with multiple sections, e.g., COMP2402 A, B & C. You may fill out one form if the exam information is the same. However, you must list all sections.

Formally Scheduled = 10 days after exam.

In Class = 10 days after last day of classes.

Take-Home = 10 days after last day of exams.

Co-op = 30 days after last day of exams.

None = 10 days after last day of classes.

Exam Availability*

Fixed Duration = a set start and end time as displayed on the schedule.

Fixed Duration within Range Exam = Questions will be available over a range of hours or days, and students will log in at a time of their choice and then complete the exam in a fixed amount of time. Please refer to the “Duration” column to confirm.

Range Exam = Questions will be available to students over a range of hours or days, and they will have access to the questions throughout the entire range (If no “duration” is listed, the exam will be open for the entire range of time.)

For full summer courses, you may request Examination Services to formally schedule your midterm exam during the June examination period. If you plan to schedule your midterm during class, please select “None” under the Exam category.
