This letter is available only to participating Co-op students who have been formally admitted to the Co-op Program. This letter confirms:- full-time Co-op student status
- work term availability
- your program of study
- a return to full-time studies following your work term(s)

For students requesting:- Reinstatement/readmission to the Co-op Program due to low CGPA, low registration, or after voluntarily withdrawing from Co-op.
- Reinstatement to the Co-op Program due to violation(s) of the Co-op Participation Agreement.
- Students wishing to request permission to enroll in a daytime course while on a Co-op Work Term.

For students requesting permission to enroll in 1.0 credits while on a Co-op Work Term. Both courses need to meet our approval of being online unscheduled or delivered after 4:00 p.m. Our office will not approve daytime courses. -
- For students not on a Co-op Work Term, to withdraw from the Co-op Program.
- For students who have the Co-op option on their student record, who no longer want to be considered for the Co-op Program.
- For students who have not achieved the required amount of Co-op Work Terms to obtain their designation, in order to facilitate a graduation application.

For participating undergraduate students who have completed or are completing COOP 1000, and wish to request a change to the work-study pattern.

For students who have found a Co-op position external to the Co-op Job Board. Requirements for a Self-Directed Position can be found here. -
For participating international Co-op students only, who have been formally admitted to the Co-op Program and require an extension of their Co-op Work Permit. -
Graduate students will use this form for the following purposes:
- Work-Study Pattern Change Requests: For participating graduate students who would like to request an optional 3rd Co-op Work Term or a change to their work-study pattern.
- Graduate Co-op Student Appeals: Submit this form to request special consideration for a unique situation or circumstance related to your participation in the Co-op Program.
- Concurrent Graduate Requests: Submit this form to request to graduate concurrently while complete a Co-op Work Term.
