FED Instructor Guidelines for Reporting Academic Integrity Misconduct

To report an alleged instructional offence, the course instructor must provide the following items for each case:

  1. Student name(s) and student number(s).
  2. Course and section(s).
  3. Course outline.
  4. Assessment question(s) where the allegation took place; and the solution(s) and/or rubric(s) for the question(s).
  5. Student’s original work, the work of any other students involved in the alleged offence, and other documents relevant to the allegation (such as on-line sources referenced in a plagiarism allegation).  Note – currently our office is only accepting electronic files.
  6. Specific details regarding the allegation; this statement should include all pertinent details regarding the circumstances:
    When and how the allegation was initially identified.
    Which student(s) do you think this student copied answers from?
    What was the website used?
