FED Instructor Guidelines for Reporting Alleged Academic Integrity Offence

Effective September 2025, to report an alleged academic integrity offence, the course instructor needs to complete the FED Academic Integrity Offence Report Form

Academic Integrity offence reports need to be sent via email to:

• For full-time faculty and instructors – FEDacademicintegrity@carleton.ca, or
• For contract instructors – the Department Chair or School Director, who will review and then forward to FEDacademicintegrity@carleton.ca.
