Internship Insurance Forms for Paid or Unpaid

Please download the below forms, fill them out and upload to the ID website (https://carleton.ca/id/student-info/internships-co-op-exchange/upoload-internship-forms/). The Student Declaration of Understanding and Agreement form is the same whether it is paid or unpaid. The student will need to complete the student section on the employer's form before giving it to the employer.
