Please note that only the Claimant Name (your name), Claimant ID (your student number), Claimant Address, event destination, event dates, purpose of claim, date, and signature have to be completed.

Please attach copies of receipts (or copies of internal chargeback forms if appropriate) for all expenditures related to this activity. These are required to process your funding application. If the funding is to be issued to a club bank account via a cheque, please include the mailing address and ensure the invoice is billed to Carleton University.

Applications received after the event has taken place will not be considered. Decisions of the committee are FINAL and are not subject to appeal. All approved expenses must have copies of receipts, boarding passes etc. for reimbursement.
