Please note that the use of this Service is restricted to academic and/or research purposes. Services may only be used by students, staff or faculty of Carleton University.

Commercial use of these Services is prohibited and includes, without limitation, use of the Services on behalf of other institutions or entities for compensation.

A Security Assessment has been completed for Qualtrics and has passed Carleton University’s Data Protection Risk Assessment (DPRA) based on HECVAT standards. 

The Qualtrics University-wide Survey Tool server is located in Montreal, Quebec, Canada.

How to Get: To request access to the Qualtrics Online Survey (new accounts only), please complete the following form:

Qualtrics Software Request

You must login with your MC1 credentials to gain access to the form.

Please note that if you have a ‘Free’ Qualtrics Account using your Carleton email address and you would like to use the Carleton Qualtrics Survey Tool instead, then the ‘free’ account must be deleted first before submitting the above form.  Survey data can be saved or exported prior to deleting the ‘free’ account.  Please contact Qualtrics Support for help/support with the ‘Free’ accounts.
Support & Resources: ITS Qualtrics Brand Administrators can provide support for account related issues, recovery of deleted surveys, data breaches, access to features (available in our Qualtrics University-wide Agreement) such as Offline App, Tango Card Integration, Table of Contents, etc.  To get help for these services or options, please contact the ITS Service Desk to submit a request.
