License Details: The Qualtrics University-wide License Agreement is renewed annually at the end of September. Yearly licenses come with a 30-day grace period.

Commercial use of these Services is prohibited and includes, without limitation, use of the Services on behalf of other institutions or entities for compensation.

A Security Assessment has been completed for Qualtrics and has passed Carleton University’s Data Protection Risk Assessment (DPRA) based on HECVAT standards.  The Qualtrics University-wide Survey Tool server is located in Toronto Canada.

You must login with your MC1 credentials to gain access to the form.

Please ensure that the Student Communication Policies and the Survey Policies are adhered to when sending surveys to the Carleton community.

Support & Recourses: ITS Qualtrics Brand Administrators can also provide support for Account related issues, recovery of deleted surveys, data breaches, access to features (available in our Qualtrics University-wide Agreement) such as Offline App, Tango Card Integration, Table of Contents, etc.  To get help for these services or options, please contact the ITS Service Desk to submit your request.
