SID Key Form

I acknowledge receipt of a key(s) indicated above. I understand and comply with the conditions set forth by Carleton University for holding a key:

1. I will not lend this key to anyone;

2. I will not duplicate this key:

3. I will not tag or otherwise identify this key as providing access to SID spaces;

4. I will be entirely responsible for the security of this key. I will return this key when I am no longer in the position stated above or when SID requests its return;

5. I will pay the key replacement fee (eShop min. cost is $50 plus $2 per key) for any lost key(s).
