Key Return Appointment Form

To return your keys, please schedule an appointment with the Department office using the following form. Deposits will be refunded on the original payment card upon verification.

Select a day between Monday and Friday. Note: the University is closed on Saturdays and Sundays.

Select only one time slot for you to return the key. if you are unable to return the key in the time slot you have selected you will need to reschedule.
