Effective October 15, 2021, all individuals who are coming to campus and/or participating in an in-person Carleton activity, must be fully vaccinated and provide proof of vaccination to Carleton University. Please note that individuals are only considered fully vaccinated if they received their last dose at least 14 days before the date of their attestation, and recall that until you are fully vaccinated, you will not be able to access the Carleton campus or other designated settings.
You must make arrangements to complete your vaccine series prior to coming to the Carleton campus and/or participating in an in-person class or activity.
Booking Your Vaccine Appointment
Students can book an appointment for their COVID-19 vaccine (3rd mRNA, partial or full vaccine series) by calling Ottawa Public Health at 613-691-5505 or contacting a select pharmacy. Drop-in vaccinations are also available at any pop-up clinic or neighbourhood vaccination hub in Ottawa during their hours of operation.
For students with the University Health Insurance Plan (UHIP), SunLife is advising that you bring your UHIP card, 3 pieces of ID and proof of your address in Canada to your appointment. If you have concerns about receiving a third dose, please contact Health and Counselling Services to make an appointment with a health care professional.
Registering Your Vaccination Received Outside of Ontario with OPH
If you have received a full series of a vaccine approved by Health Canada outside of Ontario or Canada, you must provide your proof of immunization to Ottawa Public Health (OPH). Students are encouraged to upload their vaccine documentation using the COVID-19 Out of Province Dose Documentation form to notify Ottawa Public Health of your vaccine administration. Documentation of the full vaccine series must be provided in English or French (or a certified translation into English or French).
Please note that at this time the Provincial system does not provide receipts for COVID-19 vaccines received outside Canada. Therefore, you will continue to use your original vaccine receipt in Ontario.
