Dependents are covered under the plan until they reach the age of 21.
If your dependents are registered full time in school they will be covered until age 25. Proof of registration must be forwarded to the Benefits Coordinator at the beginning of each school year to ensure coverage continues.
To enroll or change benefits information, fill in a Group Coverage Change Form and submit it to Benefits Services in Human Resources.
Upon hire or loss of benefits coverage from another provider, you have 31 days to enroll in the Carleton plans. Late enrollment for any other reason will require that you provide evidence of insurability and will limit your dental benefits to $250 per person for the first year.
You can access your pay stubs through Carleton Central, under the ‘Employee Services’ tab. Select ‘Pay Information.’ Then, click on ‘Direct Deposit Advice,’ and select the year. From here, click on a pay date in red to bring up earnings and deductions for that payment.
Once you have submitted your request, you will receive an acknowledgement e-mail from Human Resources. If you receive this e-mail and you did not request the change, please contact Human Resources immediately.
To change your direct deposit information in person, visit Human Resources in 507 Robertson Hall to change your personal banking information. Bring with you:
A void cheque or a Direct Deposit Authorization Form (available from your bank)
Photo ID (examples: Driver’s License, Passport, Campus Card)
