Only students who are eligible for a practicum should be filling out this form. Please ensure that you have completed the necessary prerequisites before you submit your Practicum Application Form to the School. If you are unsure if you meet the requirements, you can review:

SOWK 3600 – Part-time option taken over two consecutive terms (Fall-Winter only).
SOWK 3601 – Full-time option, new third year entry students only (Winter only).
SOWK 4600 – Full-time option taken over one term (Fall or Summer only)
SOWK 4601/4602 – Part-time option taken over two consecutive terms, must choose terms.
