What forms do I complete as a new hire?

As a new hire to Carleton you are required to complete the Employee Documentation Package that includes the following forms. Please submit the forms in the order below:

  1. Employee Information Form
  2. TD1
  3. TD1ON
  4. Confidentiality Agreement
  5. Mandatory Training
  6. Banking information (void cheque or direct deposit advice)

Additional documents for temporary residents (international workers):

  1. Letter of confirmation of social insurance number
  2. Copy of valid immigration document indicating eligibility to work in Canada (work or study permit)
