If you are sick, and unable to work, you must contact your immediate supervisor prior to the start of your work day, so s/he can make cover off arrangements.

Yes, there is no legal restriction against collecting Date of Birth. We collect it for confirmation of eligibility to work, benefits, and pensions.

It is the worker’s responsibility to report any workplace hazards to their immediate supervisors. A hazard is defined as any practice, behaviour, condition, thing, situation, or a combination of these things, having the potential to cause injury or illness to a person or damage to property or equipment. In a workplace incident, no injury or illness occurs. An incident is defined as an event that could have resulted in an illness or injury if a slightly different set of circumstances had transpired. Examples of this can include: fires, explosions, spills, etc. Incidents like workplace injuries still need to be reported to the workplace supervisor. By definition, a workplace injury is classified as any illness, injury, or acute psychological trauma caused by physical, chemical, or biological hazards in the workplace.
